Disqualification or Excuse

Requests for excuse or disqualification must be submitted in writing. To request an excuse, follow the instructions and complete the portion of your Summons concerning “Request for Excuse.” If you are not qualified to serve as a juror according to the Qualification Statements on the back of the summons, you must notify the court by completing the “Disqualification Statements”.

Once completed, return the request by mail or hand carry it to the court. The jury office must receive requests at least five working days prior to your scheduled service. You may also return you request by fax to the number located on your summons. You will be contacted if your request is denied.
